
How a sustained release type medicine is prepared?
There are many ways to design a sustained release drug delivery system. However, you did not state the route of administration. Are you looking for an oral or parenteral (injectable) medicaiton? There are over 90 different companies that have designed novel technologies for controlled/sustained release oral medicaitons and over 30 companies developing sustained release injectable technologies.

For a sustained release injectable, one common method is to encasulate the drug in PGLA microspheres as a depot (see leuprolide depot — called Lupron and Leuplin).
For oral drugs, one method is to utilize an osmotic mechanism to provide pre-programmed, controlled drug delivery to the gastrointestinal tract. One technology comprises a polymer membrane with one or more laser-drilled holes surrounding a core containing the drug or drugs, with or without osmotic or other agents.
Alternatively, another oral technology uses a multiple dose system containing a large number of microparticles, on the order of 5,000 to 40,000 microparticles per capsule or tablet, depending on the specific formulation. The microparticles are released in the stomach and pass into the small intestine, where each microparticle operates as a miniature delivery system, releasing the drug at an adjustable rate and over an extended period of time by means of osmotic pressure.
A third technology was developed as simple monolithic matrix systems. These approaches use conventional tableting technologies to form swellable, erodible matrix tablets, caplets, or capsules that can potentially yield first-order, bimodal, and zero-order drug release profiles up to 24 hours. In addition, any combination of soluble, highly soluble, insoluble, low drug dose, high drug load, and drug combinations can be easily formulated with these technologies. The self-correcting matrix delivery system uses a variety of ionic and non-ionic excipients and additives such as electrolytes, amino acids, gums, and polymer(s) to modulate the swelling and erosion kinetics and control drug release from hydrophilic matrices over prolonged periods. In these systems the polymers and additives tend to preferentially hydrate and have the potential to influence peripheral hardening of the gel matrices via salting-in or salting-out phenomenon with progressive shift toward the central core (thus controlling erodability and release modulation). The release rate from these systems is not adversely affected by ionic strength, pH, hydrodynamics, and other physiological parameters of the gastrointestinal tract.

Corticool gel (1% hydrocortisone), which has a higher hydrocortisone solubility compared to other creams and lotions, showed a significantly higher drug diffusion rate through all of the three membranes. The Corticool gel exhibited better hydrocortisone permeation than the Prescription hydrocortisone cream 2.5%. Cortizone-10 ointment (1% hydrocortisone) showed a very low hydrocortisone permeation through all of the three membranes. It is predicted that the same comparative behavior would be observed in vivo on applying these formulations. A novel biostable three-dimensional porous collagen scaffold for implantable biosensor. $49.99 Diabetes is a chronic metabolic disorder whereby the body loses its ability to maintain normal glucose levels. Despite of development of implantable glucose sensors in long periods, none of the biosensors are capable of continuously monitoring glucose levels during long-term implantation reliably. Progressive loss of sensor function occurs due in part to biofouling and to the consequences of a foreign body response such as inflammation, fibrosis, and loss of vasculature.;In order to improve the function and lifetime of implantable glucose sensors, a new 3D porous and bio-stable collagen scaffold has been developed to improve the biocompatibility of implantable glucose sensors. The novel collagen scaffold was crosslinked using nordihydroguaiaretic acid (NDGA) to enhance biostability. NDGA-treated collagen scaffolds were stable without any physical deformation in the subcutaneous tissue of rats for 4 weeks. The scaffold application does not impair the function of our sensor. The effect of the scaffolds on sensor function and biocompatibility was examined during long-term in vitro and in vivo experiments and compared with control bare sensors. The sensitivity of the short sensors was greater than the sensitivity of long sensors presumably due to less micro-motions in the sub-cutis of the rats. The NDGA-crosslinked scaffolds induced much less inflammation and retained their physical structure in contrast to the glutaraldehyde (GA)-crosslinked scaffolds.;We also have developed a new dexamethasone (Dex, anti-inflammatory drug)-loaded poly(lactic-co-glycolic acid) (PLGA) microspheres/porous collagen scaffold composite for implantable glucose sensors.
